Health Care Directive
The advance health care directive allows you to choose a person to act as your agent for making medical decisions on your behalf when you are not able to. It also allows you to specify in advance your specific instructions for end-of-life care, especially with regards to life-sustaining treatment. The directive also directs your agent as to your wishes for disposition of your remains, including your wishes with regard to organ or tissue donation. The directive also contains the necessary language which will authorize your agent to gain access to your medical records under federal (HIPAA) and state (CIMA) law.
